Find the hidden chest in Rane's town scene and take the Qualis Gem. You do not need to hear the chest rumor first.
The chest initially holds one gem. Later rumors can replenish other items, but not another gem.

One time
Red Brotherhood Hideout
Ask tavern travelers for rumors, buy the hideout map, then open the chest near the cave entrance.
The gem is in the scene chest, not post-battle loot. It does not replenish after being taken.
Repeatable if the spawn returns and capture succeeds
Capture a Unique Spawn leader
Defeat a Unique Spawn with a capturable leader, capture that leader, and choose a Qualis Gem in the prisoner dialogue in exchange for release.
This does not apply to every army. Eyegrim and the Three Seers use a special post-battle reward roll instead of prisoner dialogue; see below.
Recurring chance, random prize
Noldor tournament in Elacrai
Unlock Elacrai, enter a Noldor tournament, and win. A Qualis Gem is one possible prize.
The community guide reports roughly a 20% gem prize chance; winning does not guarantee a gem.
